Automated BookMarking

I have thousands of PDF books that need Table of Contents bookmarking and in need of an automated method without opening each document. The TOC rarely is on the same page or is named in many ways (Table of Contents, Contents, TOC, Whats Inside). Also, the chapters vary (Chapter 1, Section 1, One, Part 1 or By Title) and to make it more difficult the destination page may be number differently that the PDF page numbering (Chapter 1 = Page 21 of PDF). The solution can be in any platform from Mac, Windows, Unix, Linux, ect...
