Checkboxes need to show up, but not when PDF is sent?

I have several checkboxes on the main pages of my PDF that are coded to show/hide other forms. I need these to be visible so employees are able to check them and choose the forms they need. Although, this PDF will then be sent to clients. I know that you can make the boxes visible, and yet invisible when printed, but the document will not be printed; it will be sent out via email. They do not have Acrobat on their computers, so they cannot print to PDF (no other print to PDF applications are on there either). Is there a way to do this? Better yet, is there a way to somehow save certain pages of the document; i.e., having the checkboxes on the last page and saving everything but that last page to send?

You can select "Document > Extract pages" to select a range of pages to a new document.

Maybe I should have clarified a little more... I need the checkboxes to be there when it will be used in Reader. The users will not have Acrobat installed, and I don't see an extract pages option in Reader.
