Form - Approval Workflow Process

I am looking for a solution with the following requirements:

1. Design a form for on-screen input, such as a Time Off Request
2. Ability to send that form for approval (not review)
3. Ability of Approver to digitally sign the form
4. Ability for initial person to check on the status of the form.

I have Adobe Acrobat Pro 9.

The designing of a form for on-screen input is simple, however I cannot figure out a workflow method to route / approve / check status of the form.

There apparently is a feature in Version 9 referenced in the documentation that would be perfect, called "Approval by Email" under the Comments tab, however it is only available for Chinese, Japanese or Korean versions.

Looking for advise on other methods to achieve these basic requirements. There are other software vendors that do forms design with workflow however they are expensive.
