Is it possible to highlight a portion of a pdf document that has not been OCRed?

I do believe that there has to be some "renderable" text to "stick" the annotation to.
The OCR output "hidden text" serves that purpose.

any numer of the 'comment and markup' tools can be adapted to highlight an image PDF. just experiment a little with the 'shape' tools and with the 'text box' tool (without text) and you'll find your own answer.
