Shared Review of Forms

I briefly searched for this topic with no hits.

I created a form from a word document and added one dynamic control: a button to clear all the form fields. The remainder of the form is composed of text fields and checkboxes.

I am using Acrobat 9 Pro (Version 9.3.2). When I use the 'Send for Shared Review...' command, I send to reviewers in a message as a link.

The copy of the document that reviewers open, doesn't display the form fields and enable reviewers to use them. I performed an 'Examine' document and it finds all the form fields in the Navigation Panel, but I can't figure out why they aren't visible. I checked all form fields' properties, and all are visible. I can use the form before I send it for review, but somehow sending it for shared review is changing something about the display of the form fields.

Anyone seen this and resolved it?

Are you distributing the form on Acrobat.com, email, or an internal server? Also, did you use Designer at all during the creation of the form?

I'm using an internal server to host the review copy. And I have not used LiveCycle Designer at all. Just using the tools in Acrobat.

If you're using a web server folders, this may be causing the problems as these are not available for form distribution.

What product are the recipients using to view the PDF?

Acrobat and Reader.

I am sending for Shared Review, intending to enable viewers to add comments & markups, but if they can't experiment with the fields, they can't comment & markup about them. I am not releasing for use.
