I'm using Acrobat Pro 7. I created a form by first converting a MS Word 2007 document to PDF using Adobe PDF printer function. I then created text fields as appropriate. I've test the form by entering data and then saved an individual PDF file for each entry. When I insert each individual document so that I can create a single PDF file each page has the same data as the first file/page.
What is causing this and how can I fix it?

You need a different field name on each page. Text fields with the same name share almost every property. The value property is one of the shared values so the value for the form field on page one is assumed to be the value for all of fields with that field name.
How can I do that for about 100 documents/PDF files?

This behavior is the same in all versions of Acrobat, and (if I'm not mistaken) in LiveCycle as well. It's not a bug, it's a feature...

If you don't need the fields to be interactive after you've combined them into a single document, you can first flatten the fields, which converts their appearances to regular page contents, removing the fields. So when they are combined, there won't be a problem. Do do this, you can use the doc.flattenPages JavaScript method in a batch process to flatten a collection of documents. You can then combine them into a single document.
