FAILED TO ADD THE RETURNED FROM TO THE DATA SET FILE : c:\pb\july2011.pdf
  
Getting this error on a procedure we have been using for several years. The forms come back to one machine and when we try to add form to existing data set we get this error. Have tried creating new data set and still get the same error.
 
Any ideas ?

Have you tried a Save as and overwriting the file on both the dataset and the returned form? You may have a damaged file.
