Adding multiple copies of the same form in one file?

I am new to creating forms and just used the Run Form Field Recognition feature. It's great! I then went in and adjusted the form fields, set the font size, etc. So it's working just fine.

I'm only having one problem. At the bottom of the form it says "Duplicate this form as needed." Meaning, a person needs to fill out the same form multiple times for different pieces of equipment (or let's say it was one form per family member, etc.).

When I add a second copy of the form (Document / Insert Page) suddenly the form fields are populated with data from the first page. I understand this is because the fields have the same name, but how can I get around this (or can I)? I need to be able to enter unique data on the same form but keep it in the same file.

Look at the use of "templates" in Acrobat forms JavaScript. This method allows for the automatic renaming of fields.
