Anybody know how to put an opt-in form in a PDF?

I would like to embed an aweber form into my PDF. For some reason, the older version of Acrobat would create a pdf from the html code that worked. However, during the conversion it messed up the position of my graphic and opt-in box. When I tried using 9 pro extended, everything was placed correctly, but I got an error when I tried to submit the data to aweber. Below are the files that I created.

Also, I was wondering if it would be possible to create an email form in LiveCycle that would the same thing as the aweber form?
