Auto Save Form

Hi,
I have a form which I need to save on the click of a button. I need this form to auto-save with a filename based on certain text fields within the form. For example, based on "First name-Last name". Or even based on one text field will do. Is it possible?

It would be a bonus if I could also automate the save location.

Any help would be greatly appreciated.
