Automate addressing

I work for a non-profit and I have a two page PDF questionnaire with 290 fields. How do I automate a process to create 5,000 copies with each recipient's name, address, city, state and zip code?
Any help is greatly appreciated.

This is a process called variable printing. Printing doesn't necessarily mean actually printing, but act of merging data into a PDF Form. There's plenty 3rd party software out there doing this, but if you can program you could also do it with Acrobat JavaScript or a custom VB program.
