Is it possible to create pdf form in which a box can be created at the bottom for a physical signature- not a digital signature- a physical signature made with a non electronic stylus on a touchscreen laptop. Much like say using the pencil tool and drawing your name with a mouse.

This would be used by nurses in patient homes in which the patient is signing the screen on the form.

If so what version do I need to make this happen?

There isn't such a feature at present. The closest you can do with Acrobat is the draw the box on the form and use the pencil annotation tool to draw on the document (and hope people stay within the box).

There are companies that make extensions that support signature pads that could be a solution also. See http://partners.adobe.com/public/developer/security/index_security_partners.php
