checkbox line thickness inconsistent

I have a document with 200+ checkboxes. For some reason, the line thickness is not consistent despite identical property settings. The result looks sloppy - and cannot be submitted to the client.

I am able to cut the offending box and replace it with a copy - but i'm hopeful there's a better and easier way to promote consistency.
