Create Form, save and Email entry

I would like to create a form using Acrobat Pro 9.
I want to either email this PDF form or save to my nework.
Have the end user enter the data, save the pdf and manually email the file.

This completed form will not always go to the same email address.
This is a preformace review form so the information needs to be protected.

Can this form be completed by the end user, saved with data in tact as read only?
The end use would then email this file manually.
I would like the saved form (once data is entered) to be saved so the data does not change.
The reviewer should get a read only PDF file with all the responces.

This is my first attempt at makeing forms please let me know if this is possible.

It is just not possible to do?
All I really want is to make a form and have someone enter data, then save the data.
I dont want to use a server or submit to a set Email.
These forms would be public use on my network.
So the data entry should be manually sent, not programmed.

Or should I just use word templates?
