Creating duplicate pages in a PDF Form

I am currently using Acrobat 9Pro and creating a form. the individuals completing the form will need to complete page 1 with basic information, and give me a list of account numbers. They will then need to answer a series of questions for each account. They could have 1-5 accounts. I would like to have just 1 page show up for them to complete and then have them hit a button to add a new blank sheet, if they have multiple accounts. Is this possible?
 
Also, is there any way to make an if - them scenerio? If they check the box then they answer the questions in Section 2. If they do not check the box, then they answer Section 3?

This is possible using a dynamic LiveCyle form. But you'll need to learn something about designing and scripting LiveCycle forms.
