Creating A PDF form / Database

How can I take word document and create a PDF that will allow users to enter data that will go over to a database? I have Adobe Pro 7 and 8. Thanks.

You can always do it the old fashioned way and do mail merge with Word. Then PDF the merged Word files.

Other than that, use LiveCycle Designer. See Designer Help on how to convert Word files to PDFs and how to link database fields to a PDF form.

Well, that's a big question. The easy way is to hire a consultant/developer to do the work for you. But if you want to do it yourself you'll have to learn more about the whole forms and data workflow. You can start by learning how to create forms in Acrobat and LiveCycle Designer. Read all the documentation on connecting to databases and check out the sample files provided on the Adobe user to user forum and [url=http://www.adobe.com/devnet/]http://www.adobe.com/devnet/[/url]

Tigereye: Please try omniform 5.0 or 5.0 Premium..Works as a rapid application development tool for testing usability for forms. All the field data can be exported to a spreadsheet. it also has tools for calculation etc. It takes very little time ( less than 15 minutes if you already have the word document ready) to create an up and running non-relational database for small jobs. This has an upper limit of data sie of 4.5-5 MB( typically 4-5 thousand records can be stored) . Of course, you can always link this form to another database like access or create XML output
Omniform is my first choice for rapid form deployment, particularly to test usability of a form-Database. Acrobat falls short in many areas. The Version 9 has some automatic field recognition capabilities ..but comes nowhere close to Omniform 5.0.
Nuance unfortunately is not very good in developing cutting edge products. they have frozen development on Omniform...so you have to use windows XP or lower..not win 7.
