Creating a 'special' pdf form

Hello

I am trying to create a 'speacial' PDF form but I can't find a way to do it.

Firstly, the form should have a reference letter structure, with a top part looking like this:

// REFERENCE REQUEST
//Name
//Address
//Address
//Address
//Date
//
//
//Re: (worker full name) (date of birth) (ID number…if applicable)
//Job Profile (title/position applied for)
//
//... We would be grateful if you would reply to the following questions regarding this applicant and provide in confidence any information of which .....
// FORM BODY

Alright. Now the problem is that I need this form for every worker, so I can't edit it in designer/Acrobat for each and then save it as a pdf.

I need a method to change the Name, Address, Date, Worker name and date of birth and then send it.

It would be OK for me to make " Name, Address and the other " form fields, so I can complete them before sending it to the manager to complete the rest, but I don't know how to lock them, so the manager can't make any changes.

I have workers' details in an Access 2003 database using ODBC, but I think it's not possible to pull up data from the database when creating the form.

Can someone help me with this?
