Creating Templates for input by others

I have a client that wants to create a PDF template so that their brokers can SIMPLY fill in the information. Basically, I was just thinking of a form so they can fill in the fields, but then they would have to save the form and then send it to their potential clients. There are also some images that will need to be dropped into the document.

Here is an example of what the end product would look like.

So you can see the information that would need to be filled in by each broker, including some images.

Does Acrobat Professional 8 support this type of application? If so, some guidance on how to go about doing this would be greatly appreciated.

You must "Enable usage rights in Adobe Reader" (in the "Advanced" menu) if you want the end-user to be able to save the form with Reader.

The end-user can't add an image using form fields in Reader, but this can be done by using the file attachment tool (or the file attachment navigation panel).
