Creating yes/no radiio buttons

How do I make a series of radio buttons for yes and no for a form? I created one and wanted to copy it for the rest of the yes/no places on the form but I am having trouble doing that.

Make the first set of radio buttons and name them the same (button1 and button1). Click the Options tab and set the export value for the first button to Yes and the export value for the second button to No.
Continue this process for the remaining buttons but give each set a different name.

Hi,

After i creating the yes and no radio button, i would like to save the result into my MS SQL. May i know how and what is the java script for this ?

Are you using Acrobat with ADBC or LiveCycle Designer?

The Javascripts are different for each.
