Distributed Forms Not Populating in Portfolio

I have distributed and received forms back, however the received forms will not populate in the portfolio. I get a message that I have a response but nothing shows up. HELP!

My Product Information:
Acrobat Pro 9.2, Windows

How did you create your form? In Acrobat or LC Designer? What the form originally created in Word?

I used Word to create this form initially. I am trying to use LC Designer as we speak.

Was this Word 2007? Any success with the Designer form?
