how can i create text boxes to edit fields on a pdf document?

Does this PDF already have forms fields? If so then use either the TEXT FIELD TOOL or the SELECT OBJECT TOOL and double click ...

Otherwise go TOOLS/FORMS/SHOW FORMS TOOLBAR .. Add your fields to the document
