Field group template

I'm using Acrobat 9 and need a way to save a template of a group of fields, namely SIGNATURE, PRINTED NAME, DATE, TIME. I would like to be able to open an existing PDF and paste this template near the bottom of a specified page.
 
As a side note, the SIGNATURE field must be much larger than the other 3 fields. The larger field must be centered to the left of the smaller 3. The 3 smaller fields will be stacked vertically to the right of the larger field.
 
Any help would be greatly appreciated.

It's possible to create a script to automatically add the fields to a certain position on the page.
Another option is to create a blank template page with the fields, and then use the Replace Pages function to replace the blank page with your actual pages. The fields will remain in tact if you do that. Just remember to save the file under a different name afterwards.
