A scanned PDF form can be used as the basis for an interactive form. Leave the default option to apply OCR to the file as it is scanned, which allows you to make modifications in your fields' labels if necessary.

If you are working in Acrobat 8 Professional in Windows or Mac, use the Forms tools to insert and configure the fields. Try the new Form Field Recognition process to have Acrobat insert the fields for you. Your success will vary depending on the structure of your form and the quality of the scan. Check Ted Padova's article on Form Field Recognition for tips on how to improve the quality of your results.

If you don't need the form to be interactive, use the Typewriter tool to type text responses for fields.
