Filling form from web application

Hi everybody.
I have a web application (php + mysql) and I would like to fill a pdf file with data extracted from the database. Of course, data are each time different according selected item.
As far as I read it is possible to achieve it by javascript and an HTTP REQUEST.
I wonder if someone knows where to find code examples about this subjects.
