Form Capability Question

We have a client that has asked us to find a solution for them, and I believe Acrobat may be a good way to do this. They have multiple users filling out different (varying length text) forms at different times, and each entry needs to be signed/locked as a compliance requirement. It deals with caregivers in a nursing home environment, where up to six different folks will fill out text fields through out a day, and when each one completes their field, it must be signed and unchangeable.
 
I'm pretty sure that if it IS doable with Acrobat, it's beyond our capabilities and would need more expertise to handle it, but is this a job that's reasonable to do with this toolset?

There's no easy way to do a real sign and lock for a single set of fields. But, you can make a group of fields read only. And it can be done from a signature field. However, using multiple signatures on a PDF is problematic.
They don't play well with one another. But still, this technique will work. You could also use something else besides a digital signature to initiate the lock.

Try it yourself. Create a form with a digital signature. In the properties for the signature field there is a section for locking selected fields on signing.
