Form Distribution from Local Intranet

I have a form I want to distribute on our local intranet. I want to link to it on our internal website. When someone clicks the link they fill out the form and submit it. And the form will be stored on our local intranet. For the life of me I cannot figure this out! The submit button I put at the bottom of the form does not send it to the response folder. What am I doing wrong?
 
Any help would be appreciatted.

You will need a program (script) on the web server to submit to. The program would do the job of taking what is submitted and saving it somewhere. This will require some programming unless your server already has some sort of file upload facility. A submit action cannot be used to directly copy a file from a client machine to the server.

OK, is there a way for them to fill out the form and submit the response's to the server without using a submit button?
