Form Field Alignment - Fillable Form

I am using Adobe Acrobat 9 Standard Version 9.4.5. I have created a “Form Fill” document using an existing scanned form. I place the existing form in the printer and select “Print Fields” and Page Scaling is set to "NONE" then print. The fields do not line up with the document. Most are high and to the left.
 
I've scanned the original form at 300 DPI and 600 DPI on 2 different scanners with the same results.
 
Any clue what I'm doing wrong? I can move the fields in the editor to get it to line up when I print but when my staff uses the form to fill it in, it's not real clean, as fillable fields are located over areas of the form they shouldn't be.

This has to do with the way Acrobat takes into account the printing borders. A looong time ago, I had this issue because the client had to print onto multiple copy stock using a dot matrix printer.

At that time, we first increased the Media box of the document, and then set the Crop box so that the fields aligned properly (using the getPageBox() and setPageBox() methods in the Console). I think I created an utility to facilitate that, but I'd have to dig it out of cold storage…
