Form fields disappear when I replace pages

I have a fillable PDF document that I need to replace with updated pages. Typically when I go to the fillable PDF and select "Replace Pages", the form fields retain their format and the pages are simply replaced with the new ones.
 
This time, however, I am unable to retain the form fields. When I go to Forms >> Add or Edit Fields, the form fields are there, but I am unable to see them on my document.
 
My settings are set to Highlight Fields, none of the fields are set to Hidden, they are just not on my new document.
 
Can anyone help with this?

Not sure what you mean that they're not there. If you go to Forms >> Add or Edit Fields and they are there, then the fields are there. Can't be in Forms edit mode and not in the document...Choose the Select Object tool when you open the document and roll over where they should be.

Otherwise, maybe some more info would help us...
