I have a PDF form I'm making form fillable. I have about 10 lines in a row which I have put check boxes by. If I check one box they all get checked. How do I correct this?

Change the field name or change the 'export' value of the check box. Use of the same name makes the check box a member of an exclusionary group and they act like radio buttons with the exception that you unselect a selected box.

The same requirement applies to radio buttons.
