Form Wizard Not Recognizing Check Boxes

I'm having an issue with getting Adobe Acrobat to automatically recognize the check boxes in my .pdf forms.
 
I am using InDesign CS4 (version 6.0 and 6.0.6) to compose order forms for our products here at work and then I am using Adobe Acrobat 9 Pro (version 9.4) to make them "active" forms.
 
When we are done in InDesign I export them to .pdf forms and open them in Acrobat. When I run the Form Wizard it is recognizing only text boxes and none of my check boxes.
 
There are 3 people working on this project but only forms made by 2 of us are having this issue. If I try to convert a form that the 3rd person made it works fine and if he tries to convert a form I made he has the same issue I did. That seems to rule out something that we are doing during the conversion process and makes me think it's something that we are doing in InDesign when we create the documents but I can't figure it out. We are both using version 6.0.6 of InDesign(the 3rd person on our team is using 6.0 and having the same problem I am).
 
I'm usually pretty good at figuring these things out but this truly has me stumped. Any insight would be greatly appreciated. I have no background in graphic art or working with Adobe so please type slowly and use small words in your responses.

This is always a problem. It seems that Acrobat wants to see a small box to the left of an isolated string of text. I'm sure the issue is with how the elements are organized in InDesign. Take is look at the details. It could easily be an issue with how the elements are grouped. Are there any intervening elements, text symbols, non-printable characters?

The .pdf's actually look very similar. I would understand if Adobe was missing a few check boxes or even most of them but it is recognizing all of them in one document and none in the other document.

Here are a couple of links to two documents on our company website. The first document was written by the person who had this job before me. The second is a document that I created in InDesign. The format is visually similar but the second one will not convert.

There are Font encoding and color space differences between the two, but I can't imaging these differences would cause the differences in field detection. Perhaps there are differences in physical layout? This is more of what I would expect to be a problem.
