If you are using Acrobat 8 Pro, you can use Forms/Run Form Field Recognition. Acrobat 8 will recognize existing form fields and create them for you. You may have to edit those form fields.

If you are using Acrobat 7 Pro, you can use the Typewriter tool to simply fill out the form on a one-time basis. Or you can use the AcroForms tools to create the form fields.
