This question is very broad. There's much to creating PDF forms. You might consult the online help guide in Acrobat Pro for assistance in using the Forms tools. In addition you'll find a number of Acrobat aftermarket books out that cover creating PDF forms. Browse your local Barnes and Noble or Borders bookstore and thumb through some books to find one to your liking that explains using the Forms tools.
