How do I make a non-interactive form an interactive form?

OK, The quick and flippant answer is to... "Add form fields." However, I do not think this is what you are hoping for.

To keep the brevity of this under the size of the NYC Business to Business phone book, I would recomend investing in one or several books on the subject as well as reading through articles on creating forms, form fields, etc.
