Acrobat and JavaScript don't have mechanisms to save a data file with a specific field name. You can do this, however, by having a CGI, ASP or whatever script "catch" the data output from an Acrobat/Reader submitted to a url.
Your script then can look at the data file, find the user data in a specific field, and then save the data with that file name.
