Having trouble with getting forms filled out with Acrobat Reader 9

We have our forms on line through Acrobat Pro 8 and were doing fine until the Acrobat Reader 9 came out. Now many people can't use the forms. They can't save as or submit. They can fill the forms out 80% of the time and print, but that doesn't help us get the information upfront prior to their visit. Any idea what is wrong?????

My Product Information:
Acrobat Pro 8.1.2, Windows

It's hard to tell without seeing an example. Can you provide a link to one?

George, here is a link to the newest form I just did:
www.mydrhorn.org/docs/Nutrition Card App.pdf

Another example is one of our regular forms:
www.mydrhorn.org/docs/Neurotransmitter Questionnaire.pdf

I had researched this site earlier and it mentioned "enabling" which I have done but it still doesn't seem to help with Reader 9.

Thanks!

You've set up that first file to submit the entire PDF using the mailto protocol. The file does not have usage rights (i.e., Reader-enabled), so Reader is not able to save it or submit it outside of a web browser. In order to attach the filled-in form to an email, it must have usage rights applied. Can you provide a link to a file that you have Reader-enabled?

Both forms were "enabled" through Acrobat 8 Professional or so I thought...I didn't get anything that said it didn't work!

The two files linked to above do not have usage rights.

OK, today Dr. was unable to open his lab reports with Acrobat Reader 9. The same problem we have with our forms. We deleted Acrobat Reader 9 and just used Acrobat Reader 8...he got his lab reports open and able to save AND he went into the website and was able to use the forms.

I really think there is a bug with Acrobat Reader 9. I can try to send you the original forms before I secure them or "enable" if you think it would help? He is getting frustrated because our patients can't utilize the forms on-line and we have a lot of patients that can't just hop in their cars to pick up the forms and if we email them, they still have trouble.

As you can tell, we are getting to the end of our rope and I appreciate ANY help you can give me or suggestions I can try.
