How do I convert Non-interactive fields to Interactive

I have a form from my health insurance company that I would like to fill in name, Dr. Name, etc before printing. Can anyone tell me where I could find the procedure to do this or tell me how to do this. I am using Acrobat 9 Standard.

The Typewriter tool was designed for this kind of scenario, but the PDF has to at least allow commenting. I don't have access to Acrobat 9 at the moment, but you should be able to get more information from Acrobat's help.

Adding actual form fields is possible in Acrobat 9 Standard if the document is not protected, but this may be more time consuming than you want.

Just be aware that the 'typewriter' tool is an annotation or note and you have to make sure that you are printing 'markups, annotations, comments, etc.' or you will have an empty form. The form also has to allow commenting or editing for form fields.
