Can it be done? I don't want to put a link to the PDF document.

My intention would be to make the form a part of the presentation.

My Product Information:
Acrobat Pro 9.1.1, Windows

You could insert an image of the form with a link to the form. MicroSoft does not provide the code to decode the PDF foramt within their products.
