A little help

I am trying to set form fields for a form and am having trouble. The input needs to go all the way to the left when done but spaced evenly ovver a set area. This is confusing I know/ I have (2) samples available. # 1 is the way I want to be able to fill the form. And # 2 is the form I am trying to set up. Any help would be amazing.
