Looking for someone who knows how to design forms

I am looking for someone that could tweak some settings on a form that we have created already.
We have the form set up but it does not seem very user friendly. The users would like to be able to tab from box to box and they are not able to. They also would like to be able for the form to auto-populate information that has previously been entered in previous forms. We also are having problems getting the check marks to work properly. They have to use an X instead of just clicking on the box to put a checkmark in the field.

We are in the Los Angeles area.
