What’s the best way to design a form that is completed by numerous people at various locations? Would Adobe Workflow be the way to go in designing such a form? Do you have any sample forms that I could look at?

You can use Acrobat PDF forms in such a workflow. For sample forms, search the Internet for PDF Forms and look at the sample forms in the Deisigner folder on Windows installations of Acrobat.
