Operation Failed

I'm using 9.0.0 Professional. Created a form, enabled usage rights, but when submitting I get and operation failed error when I click on the email button. What is going on??? Thanks!

Acrobat Pro Extended 9.0, Windows

What is the error message? Did you set the email address for the submit?
