placing a smaller pdf file into a larger pdf file

I have a small (2" x 3") pdf file with a few text boxes that are to be filled out by someone else.

I am trying to cut and paste this smaller one into/onto a full page pdf file. Is this possible?

Acrobat Standard 9.1.3, Windows

What was this small PDF created from? I'm not talking about the interactive form fields that were added in Acrobat. I mean the form design, the text and graphics, where did they come from?

It is this file that needs to be modified. The form fields can be copied from the old PDF form into a new one.
