Question about blanks in combo box selections

I am not so sure even has an answer but my boss has asked me to "figure it out". We have forms we have to fill out with our patient's information and medical problems. I have a list of about 600 that I will be converting to combo boxes which is the easy part. The hard part is that almost each one has a blank that needs to be filled in after it is selected.

As seen above I have made 2 selections and each have blanks that would need to be filled in after the selection is made. Can this be done?

The example above would need to be filled in with one of the words listed in parentheses (chopped, mashed, pureed). AFTER they select the problem they need addressed with their patent they need to fill in the blank of that selection.

They usually fill in the blanks by hand after printing. Some examples of information they would write in would be: Q shift (meaning per shift), PRN (as needed), BID, TID, or QID.

I hope I am making sense.

So what's the problem? Just leave an empty space for them to fill in, either manually or using a text box if you want to allow them to do it in the file itself.

The blue selections are the blanks I am referring to. They are drop down combo/selection boxes. After they have selected one they want to be able to fill in the blank. Can I add a text box to a combo box selection?

It look like you are trying to insert the combo box in a text field.

You could have a drop down selection with the options and next to that combo box you would have a text field that additional information could be entered or another combo box with the options that could be selected. For this I would try the text field with a validation script of acceptable answers so there is no question as to what was entered.

Would this put the information that needs to be in the blanks at the beginning or the end of the line? That would be okay if there is a way to have it entered in the combo box after the selection is made.

A combo-box can be set so that the user can type in a value themselves, in addition to the pre-defined ones.

Thanks guys. I am playing with both of your suggestions and I think I can enter the prefilled information and once it's selected they can take out the lines and enter what they want in that spot...just like letting them type the value themself.
