Radio buttons filled-in when i open a document. I have unchecked the "Button is checked by defaut" in the radio button options.

And I have a reset button on my form. When I click it the form reset, all except the radio buttons.

How do I fix this so that the radio buttons are clear when I open the document and clear when I reset the form?

Double check your Reset Form action and make sure all the Radio Buttons are also set for clearing the field data.
