Radio buttons - select more than one choice simultaneously

A clinet wants to have more than one choice in a pdf form usig radio buttons. Using form wizard I can easily make onc choice but not two buttons chosen at the same time.
 
Can anyone help?

Don't use radio buttons, since they can't provide what you want. Use check boxes instead.
