Saving text field layouts

I need to create a number of pdfs based on jpg files. The finished documents would consist of the jpg with about a dozen text fields for the user to complete over the top.
The different jpgs are all very similar and the positions of the text boxes would be the same in each one.
Is there some way I can create some kind of template that includes the text boxes and then insert the jpg underneath? Or is there another solution to this problem? The originating files don't have to be jpgs - they're currently .psd files.

You can use the Replace pages feature in Acrobat to change the underlying images (your jpegs would need to be converted to PDFs). This will keep the form fields intact.
