show/hide pages in acrobat form

First off, I am a big novice at this. So when responding please be gentle.
I have a question, I have an acrobat form I'm developing for work. I have fields that show when a check box is clicked. What I would like to do is when that specific box is clicked it shows about 4 - 6 pages that correspond with the check box. There are three "choices" to use.
I know that templates need to be used but I'm a little lost on how to call the template to show when clicked.
 
Any help would be appreciated!

If it has to work with Reader, it's not normally possible. Page templates can work with Reader only if the corresponding usage right has been enabled for the document using Adobe's LiveCycle Reader Extensions. Acrobat is not able to grant this particular usage right.
