Specific issue saving PDF forms created in Acrobat 8 Pro

Hey everyone, hopefully someone can help me sort out this issue..

I've recently created a PDF survey form, and it seems to be a hit or miss with people. Some have no issues with it, others seem to have problems saving the document. I had a friend of mine mention the exact error that pops up when he tries to save :

"The document could not be saved. There was a problem reading this document (26)"

This problem popped up in Adobe Reader 8.0 on OSX in this particular instance. Any thoughts as to what might be causing this?

My Product Information:
Acrobat Pro 8.1, Windows

Could be a few things. First, did you enable the form with Adobe Reader usage rights using Acrobat 8.1 or above? I'm presuming you did. The file download might have been corrupted. Have one of your users Control + Click the Wb link and choose Download linked file and see of they experience the same problem. If so, you'll know there's a problem downloading the document on some systems.
