Submit button with questionaire, I would like to have a submit button on a PDF but only have questions ansswered if they click submit. Like these questions

Sounds like you need a popup input dialog. There are a couple of ways to go about this.

First, Read this series of articles.

Read parts 1, 2, and 5. Part 5 discusses my favorite solution, custom dialogs. But they are a pain to write code for so here is link to a third party tool for creating custom dialogs.
