Tagging checkboxes and radio buttons for accessibility in Acrobat Pro

Wondering how to properly tag checkboxes and radio buttons for accessibility in Acrobat Pro, not LiveCycle. They have been made accessible to the extent of providing button value fields and tooltips, but I'm stumped on how to tag these.

Each form field is tagged as follows:

OBJR

This doesn't work for checkboxes and radio buttons, however. Has anyone found a solution?

Thanks!

A trial file for use of PDF check box and radio buttons in an accessible PDF.

Some text to support check box usage
Use a Check box. Check in check box 1.

Check Box 1

(Provide descriptive instructions for this check box here.) Unchecked <== denotes status of check box
Some text to support radio button usage
Select “yes” or “no” from the radio buttons on the next line.

Radio Buttons:

(Two adjacent radio buttons. Left radio button is "yes". Right radio button is "no".: Yes) Unchecked <== denotes status of radio button
